2Per curiam

The Director of the Office of Lawyers Professional Responsibility filed a petition for disciplinary action against respondent, Byron L. Zotaley, for submitting an insurance endorsement form taken from another client’s file during an insurance arbitration and for failing to take remedial action after being informed that the arbitrator relied on the endorsement form as the basis for his decision.
