Federal habeas courts generally refuse to hear claims "defaulted ... in *1804 state court pursuant to an independent and adequate state procedural rule." Coleman v. Thompson, 501 U.S. 722 , 750, 111 S.Ct. 2546 , 115 L.Ed.2d 640 (1991). State rules count as "adequate" if they are "firmly established and regularly followed." Walker v. Martin, 562 U.S. 307 , 316, 131 S.Ct. 1120 , 179 L.Ed.2d 62 (2011) (internal quotation marks omitted).
