This case arises out of Defendant Provident Life & Accident Insurance Company’s 1 denial of Plaintiff Richard MacLennan’s claim for long-term disability benefits. Mr. MacLennan believes that his claim was wrongly denied in violation of the Employee Retirement Income Security Act (ERISA), 29 U.S.C. § 1001 et seq.
