Plaintiff S.M.1 brings this action under the Employee Retirement Income Security Act of 1974 (“ERISA”), 29 U.S.C. § 1001 et seq., against Oxford Health Plans (N.Y.), Inc., a/k/a Oxford Health Insurance, Inc. (“Oxford”), Oxford Health Plans, LLC (“Oxford LLC”), United Healthcare Services, Inc. (“United Healthcare”), and Uni-tedHealth Group Inc. (“UnitedHealth”), (collectively, “Defendants”).
