These cases come before the court upon the parties’ cross-motions for summary judgment. Plaintiffs are three health benefits carriers (collectively, the “carriers”) — GHS Health Maintenance Organization, Inc., doing business as BlueLincs HMO (BlueLincs), Texas Health Choice, L.C. (Texas Health), and Scott' & White Health Plan (Scott & White).
