Oscar Simmons (“plaintiff’ or “Simmons”) seeks continuation of his group life insurance coverage under the Extended Death Benefit During Total Disability provision (“Extended Death Benefits” or “Benefits”) of Group Contract No. G-41800 (“the Plan”).
