This case arose out of the death of William F. Peck (employee), claimant’s deceased husband, who was employed by Gainesville Machine Company (employer). Pacific Employers Insurance Company (insurer) provided the employer’s workmen’s compensation insurance coverage. A rather complete review of the facts surrounding the employee’s death is necessary for complete understanding of the case.
