Claimant seeks review of an order of the Workers’ Compensation Board that dismissed her request for a hearing under ORS 656.319(1) as untimely. We affirm.1

On December 26, 1988, claimant injured her back when she slipped and fell while working. At that time, Wine-hell’s Donuts (employer), was insured by Liberty Northwest Insurance Company (Liberty). Claimant filed a claim with employer and Liberty on July 14, 1989.
