On August 30, 1972, a New York State Police investigator laid an accusatory instrument before Acting City Court Judge, Joseph Mondo. This instrument, labeled an “ Information/Complaint ”, being a New York State Police form, was verified by Judge Mondo, pursuant to paragraph (a) of subdivision 1 of CPL 100.30. “Attached to and made a part thereof ’ ’ was a supporting deposition executed by another State Police investigator, sworn to before a notary public.
