BERDON, J.

The dispositive issue in this appeal is whether a change of beneficiary in a life insurance policy can be accomplished by substantial compliance with the policy requirements, as opposed to strict compliance, where the policy requires that the change of beneficiary be requested “on a form satisfactory to the *289company.” The plaintiff, Robert Engelman, is the executor of the estate of the decedent, Ella B.
