Defendant appeals his conviction for attempted burglary in the first degree, ORS 161.405; ORS 164.225, on the ground that the trial court erred in denying him a court-appointed attorney. He contends that he did not waive his right to counsel.1 We reverse and remand for a new trial.

Defendant was arrested for attempted burglary on May 27, 1987. He signed a “Security Release Questionnaire and Financial Statement,” in which he stated that he had $1,060 in assets and $3,000 in liabilities and was receiving $79 worth of food stamps and $400 unemployment compensation per month.
