On August 14, 1998, plaintiff, Kerry Whitaker, acting pro se, filed his complaint against defendant, Whirlpool Corporation, Tim Teasley, Doug Hagewood, Clara Vaughn, Debra Derby, Dr. Vaughan Allen, and Lynn England of the Tennessee Department of Labor Division of Workers Compensation.
