JOYNER, District Judge.

I. Introduction

The relevant facts of this case are not in dispute. Ronald Kimble is a member of the Teamsters Health and Welfare Fund of Philadelphia and Vicinity (Fund) and Hazel, his wife, is covered as a dependent. The Fund is a multi-employer benefit plan established pursuant to the Labor Management Relations Act, 29 U.S.C. § 186(c)(5), and the Employment Retirement Income Security Act of 1974 (ERISA), 29 U.S.C. § 1001 et seq.
