TRAXLER, Chief Judge:

Richard Johnson (“Richard”) participated in an employee welfare benefit plan (“the Plan”), that provided life insurance and accidental death and dismemberment (“AD & D”) benefits through group policies issued by American United Life Insurance Company (“AUL”).
