David Zuback (Claimant) petitions for review from an order of the Workers’ Compensation Appeal Board (Board) that affirmed the decision of the Workers’ Compensation Judge’s denial of Claimant’s penalty petition.

On October 11, 1976, Claimant sustained a work-related injury while in the course of his employment with Paradise Valley Enterprise (Employer). Claimant suffered the loss of his left arm, his left leg, and two toes on his right foot.
