Debra D. Stephens (“Mrs. Stephens”), as personal representative of the estate of Robert Stephens (“Mr. Stephens”), deceased, and as beneficiary of an insurance policy on the life of Mr. Stephens, sues Citation Corporation (“Citation”), Sarah Bentley (“Bentley”), and Reliance Standard Life Insurance Company (“Reliance”), alleging violations of the Employee Retirement Income Security Act of 1974 (ERISA), 29 U.S.C. §§ 1001-1461 (2006).
