As required for a permanency hearing, on March 28, 2006 a permanency hearing report was signed, verified and submitted to the court by a Monroe County Department of Human Services (DHS) caseworker in the above-captioned neglect matter. That report indicates that Depakote Sprinkles (also known as valproic acid and valproate), an antipsychotic drug, had been prescribed for three-year-old Desiree L.
