INTRODUCTION

Relators Ricia Johnson and Health Dimensions Rehabilitation, Inc. (“HDR”) (together, “Relators”) initiated this qui tam action, on behalf of the United States of America (the “Government”), against Defendants Golden Gate National Senior Care, L.L.C.; GGNSC Holdings, L.L.C; and GGNSC Wayzata, L.L.C.
