When appellant Carolyn Pari-Fasano’s long-term disability benefits were terminated by appellee ITT Hartford Life and Accident Insurance Company (“appellee” or “ITT Hartford”), she brought suit in federal district court alleging that the termination of her benefits violated the Employee Retirement Income Security Act of 1974 (“ERISA”), 29 U.S.C. § 1132(a)(1)(B) (1994). The parties submitted cross-motions for summary judgment, and a magistrate judge recommended that appellee’s motion be granted and appellant’s motion denied.
