Plaintiff William T. Jacobs, Jr. (“Jacobs”) filed suit against Defendants Guardian Life Insurance Company of America (“Guardian”) and Bill Jacobs Motorsport, Inc. Health Insurance Plan (“the Plan”) (collectively “Defendants”) pursuant to Sections 502(a)(1)(B) and 502(a)(3) of the Employee Retirement Income Security Act (“ERISA”), 29 U.S.C. § 1145, seeking recovery of health insurance benefits.
