At issue is La.Rev.Stat.Ann. § 22:215.12, which, for hospital, health, or medical expense insurance policies issued after 1992, prohibits denial or limitation of “benefits for a covered individual for losses due to a preexisting condition [that were] incurred more than twelve months following the effective date of the individual’s coverage”.
