Plaintiff-appellant filed a petition for hearing dated November 26, 1965, which was received by the workmen’s compensation department on November 29, 1965. The petition alleged that on or about September 28,1963, the plaintiff sustained a personal injury or disablement from occupational disease, which was caused by driving-under hazardous and fatiguing- conditions and resulted in disability to his head, back, upper extremities, and aggravation of a diabetic condition.
