Dennis Cooke filed a complaint in the Winston County Circuit Court against his former employer, the Town of Addison, Alabama, seeking workers' compensation benefits. Cooke alleged that he had suffered two injuries arising out of and in the course of his employment — one to his left eye on October 12, 1993, and another to his back on April 27, 1994.
