The plaintiff, Carol K. Hull, filed a medical malpractice complaint against the defendant, Southern Illinois Hospital Services, doing business as Memorial Hospital of Carbondale. The defendant moved to dismiss the plaintiffs complaint on the basis that the plaintiffs correlating affidavit and medical report did not comply with the substantive requirements of section 2 — 622 of the Code of Civil Procedure (the Code) (735 ILCS 5/2 — 622 (West 2002)).
