LOHIER, Circuit Judge:
A jury convicted John W. Caltabiano, Jr. on various counts of mail fraud, conspiracy to commit mail fraud, and theft of government property. The United States District Court for the Northern District of New York (Mae A. D’Agostino, Judge) sentenced Caltabiano principally to 57 months’ imprisonment. Caltabiano timely filed a notice of appeal using a form desig*213nated “USCA-2 Form A,” which we attach as Addendum A to this opinion.
