September 15, 1882, appellant issued its benefit certificate to one Fred Behlmer, husband of respondent, whereby it was provided .that $2,000 should be paid to respondent, as beneficiary, at his death. Behlmer paid the assessments until July 17, 1901, when he left his home, and was never thereafter heard from. Respondent paid the assessments until July 28, 1902, after which no assessments were paid. On July 20, 1908, respondent’s attorney requested appellant to furnish blank forms upon which to make proof of the husband’s death, and the request was refused on the ground that appellant was not…
