Following the death of Alexandra Pennington at Children’s Medical Center of Dallas (the hospital), the Texas Department of Health notified Detective Dan Le-sher of the Dallas Police Department of the death and the circumstances surrounding it. 1 After learning that the hospital had conducted an extensive internal investigation of the death, and that the investigation included statements taken from persons who were in the room where the child died, Lesher applied to the Dallas County Grand Jury for a…
