CUMMINGS, Circuit Judge.

On March 21, 1985, plaintiff, a state prisoner, commenced this action under 42 U.S.C. § 1983 seeking redress for being shot in the leg by City of Decatur police officers after committing a residential burglary. Plaintiff alleged in his pro se complaint that the police officers did not see a weapon, yet shot him, and that he was in fact unarmed. The pro se plaintiff filed a motion to proceed informa pauperis (pursuant to 28 U.S.C. § 1915) on a preprinted form supplied by the court clerk.
