Plaintiff Tina Myers commenced this action under the Employee Retirement Income Security Act of 1974 (“ERISA”), 29 U.S.C. § 1001 et seq., against Defendant United of Omaha Life Insurance Company (incorrectly named in the complaint as Mutual of Omaha Life Insurance Company).
