On December 16, 2009, Plaintiff Ratna Prabhakar, acting pro se, filed her complaint alleging that Defendant Life Insurance Company of America (“LINA”) had violated the terms of the Group Long Term Disability Income Policy (the “Policy”) issued to State Farm Mutual Automobile Insurance Company (“State Farm”).
