Proceeding pursuant to CPLR article 78 (initiated in this court pursuant to Education Law, § 6510-a, subd 4) to annul a determination of the Commissioner of Education which revoked petitioner’s license to practice medicine in New York State. H Petitioner received his license to practice medicine in the State of New York on July 1, 1969. A disciplinary proceeding was commenced against petitioner by service upon him of a notice of hearing dated August 18,1981. A statement of the charges was attached to the notice of hearing.
