RODGERS, Justice:

This is an appeal from an order of the Workmen’s Compensation Commission and the judgment of the Circuit Court affirming the order of the Commission.

*628An insurance agent, Albert J. Shoffner, Sr., filed a claimant’s original B-5, 11 form with the Workmen’s Compensation Commission, in which he alleged that he was injured in an accident arising out of and in the course of his employment with Vestal & Vernon Insurance Agency, Mutual of Omaha and United Benefit Life Insurance Company.
