OPINION

{¶ 1} This matter is submitted to this court on the record and the briefs of the parties. Appellant, Jeremy W. Dwyer, appeals the judgment entered by the Portage County Court of Common Pleas.

{¶ 2} Dwyer was indicted on February 27, 2007, on one count of failure to provide notice of change of address in violation of R.C. 2950.05(A) and 2950.05(E)(1), and one count of falsification in violation of R.C.2921.13. Dwyer entered into a written plea agreement on May 9, 2007, pleading guilty to the charge of failure to provide notice of change of address, a felony of the fourth degree. The…
