*230Ordered that the application by the petitioner mother, Kimberly Mack, which seeks leave to change the name of her son, born in 1994, from “Ka’Ron Lamar Stephens” to “Ka’Ron Lamar Mack” is hereby denied with leave to renew upon proper papers, which comply with Civil Rights Law § 60 et seq., including but not limited to an updated petition setting forth current facts upon which the requested relief may be granted, and proof of proper notice to the child’s father; and it is further ordered that the petitioner shall serve a copy of this order, as well as a…
