Plaintiff, Sherry F. Robertson, seeks legal and equitable ' damages from Defendant, Standard Insurance Company, under the Employee Retirement Income Security Act of 1974 (ERISA), 29 U.S.C. § 1001 et seq. Plaintiff received long-term- disability (LTD) insurance benefits and a waiver-of-premium of a life insurance policy benefit from Defendant from ‘September 4, 2012 through October 18, 2013.
