OPINION

BAKER, Judge.

Appellant-defendant Frontier Insurance Company (Frontier) appeals the trial court's denial of its motion to correct errors. Frontier contends that the trial court improperly entered a judgment of forfeiture and imposition of late surrender fees against it because the court failed to meet the statutory notice requirements regarding forfeiture.
