LPER CURIAM.

This disciplinary matter arises from one count of formal charges filed by the Office of Disciplinary Counsel (“ODC”) against respondent, James David Turnage. Respondent has been disbarred since 2002. In re: Turnage, 01-2585 (La.9/13/02), 826 So.2d 546 (“Turnage I").

UNDERLYING FACTS

On July 14, 1995, respondent settled Jacqueline Anderson’s personal injury claim for the sum of $4,036.50. On July 19, 1995, respondent accounted to Ms. Anderson for the settlement proceeds, deducting the sum of $1,382.00 for payment to The Ebrahim Clinic, Ms. Anderson’s…
