I. Introduction
Plaintiffs Daniel and Geri Krauss are participants in an employee health insurance plan offered by defendant, Oxford Health Plans, Inc. While covered by the Plan, plaintiff Geri Krauss was diagnosed with breast cancer and underwent a bilateral mastectomy and bilateral breast reconstruction on May 13, 2003.
