BLACKBIRD, Justice.

The parties will be referred to as they appeared before the State Industrial Court. Claimant asserts in her Form 3 that she sustained injuries to her back and side while working for the respondent as a grinding machine operator on April 24, 1968. She filed the Form 3 in the State Industrial Court on September 10, 1968.
