Appellant Diana Foster is a retired teacher insured through the Teacher Retirement System of Texas (“TRS”), a state agency. Pursuant to a contract with TRS, the insurance plan was administered by Aetna Life Insurance Company and Aetna Health Management, LLC (collectively, “Aetna”). In late 2003 and early 2004, Foster was prescribed intravenous immune globulin infusion therapy (“IVIG”).
