BOYD, Justice.

Appellee, Julio Sanchez, Jr. (Sanchez), was arrested in Lubbock County on July 27, 1988, for driving while intoxicated. He refused to take a breath test or offer a blood sample. In compliance with Tex.Rev. Civ.Stat.Ann. art. 6701Z-5(2)(d) (Vernon Supp.1989),1 the arresting officer reported that refusal to appellant, Texas Department of Public Safety (the Department). Upon receipt of that report, the Department sent Sanchez a notice of license suspension as required by article 6701Z — 5(2)(f).
