According to the affidavit of H. Dale Cook, Chairman of the Appeals Gouncil and Director of the Bureau of Hearings and Appeals, Social Security Administration, Department of Health, Education and Welfare, the plaintiff herein filed an application for a period of disability and disability insurance benefits under the provisions of the Social Security Act (hereinafter Act), Title 42, United States Code, Section 401 et seq., on February 2,1970.
