OPINION OF THE COURT

Respondent Paul Chazan was admitted to the practice of law *324in New York by the Fourth Judicial Department on September 10, 1976, as Paul M. Chazan, and at all times relevant to this proceeding maintained an office for the practice of law within this Department.

On or about October 15, 1997, the Departmental Disciplinary Committee served respondent with a notice and statement of charges alleging that respondent forged his client’s signature and falsely notarized the forgery, in violation of Code of Professional Responsibility DR 1-102 (A) (4) and (5) (22 NYCRR 1200.3), and…
