Two health-care providers — Dr. Louis Peterson and Riverview Health Institute (“Riverview”) — bring these actions on behalf of certain of their patients against UnitedHealth Group Inc. and various of its affiliates (collectively “United”). United acts as the administrator for numerous health plans governed by the Employee Retirement Income Security Act (“ERISA”), 29 U.S.C. 1001 et seq.
