POSNER, Chief Judge.
This is the second round of appeals in an antitrust case brought by a health insurer against a medical clinic. See 65 F.3d 1406 (7th Cir.1995). The defendant, the Marsh-field Clinic in rural Wisconsin, is the dominant provider of medical services in the north-central part of that state; indeed, despite its remote location, it is one of the largest medical clinics in the nation, comparable in size to the Mayo and Cleveland clinics.
