We gather from the record before us that the defendant association, about December, 1920, issued to the plaintiff, one of its members, a combined beneficiary certificate in the maximum sum of $1,000.00; the insured being given the option under certain conditions, of surrendering his policy for cancellation and receiving in settlement thereof, as permanent total disability benefits, the sum of $500.00.
