Plaintiff-Appellant Nancy Koehler appeals the district court’s summary judgment dismissing her suit to recover health insurance benefits under an employee benefits plan governed by the Employee Retirement Income Security Act of 1974 (“ERISA”), 29 U.S.C. §§ 1001-1461.
