Appellant Blue Cross and Blue Shield of Alabama (“Blue Cross”) appeals the district court’s grant of summary judgment in favor of appellee Kevin L. Lee (“Lee”). We reverse.

Lee, an employee of Associated Doctors Health and Life Insurance Company (“Associated Doctors”), participated in Associated Doctors’ group health care plan established pursuant to the Employee Retirement Income Security Act (ERISA), 29 U.S.C. §§ 1001 et seq.
