FACTS AND PROCEDURAL HISTORY

In this workers’ compensation dispute, claimant, Joseph J. Mike, III, filed a disputed claim for compensation alleging that he injured his lower back while lifting paint buckets during the course and scope of his employment with defendant, Bob’s Painting, on July 26, 2004.
