Plaintiff Harold L. Leonard (“Dr.Leonard”), doing business as The Leonard Clinic of Chiropractic (“the Clinic”), 1 brought suit against defendant Educators Mutual Life Insurance Co. (“Educators”) under the Employee Retirement Income Security Act of 1974, 29 U.S.C. § 1001 et seq. (“ERISA”) to recover unpaid medical benefits owed under a group insurance policy.
